On top of the fee, you pay the actual cost of employment: gross salary, employer contributions (INPS), injury insurance (INAIL) and TFR. The 13th and 14th month are spread pro rata over the monthly invoices and the TFR is paid monthly into the employee's chosen fund, so no lump sums build up. See the calculator.
Billing terms: one month's estimated cost as a deposit, plus the first month paid in advance; each month is then invoiced on actuals, with the payslip summary attached.
